General Equipment (Non X-Ray) Handover Form

The general equipment handover form can be used as part of a formal handover procedure for medical equipment that does not produce ionising radiation, which should use the Radiation Controlled Area and Equipment Handover Form.
The general form can be useful to ensure all parties understand who has control of the equipment and location. This can be critical, especially when the equipment or location may pose a hazard, such as MR systems due to the strong magnetic field and cryogens used.
The form follows the same 3-stage approach as the radiation controlled area form, so will be familiar to many imaging departments:
- An exchange of information between the Customer and the Company Representative.
- Documentation and exchange of information regarding the work completed by the Company Representative (e.g. field service engineer).
- Documentation of the Customers decision to return the equipment to use.
